What this website collects, and who receives it.

The contact form.

The form on the home page collects one field: an email address.

Submissions are sent to Formspree (formspree.io), a third-party form service, which receives the address and forwards it to SurveyArk Inc. Formspree handles that data under its own privacy terms.

SurveyArk Inc. uses the address to reply to you, and keeps it until you ask for its removal.

Email.

Mail sent to info@surveyark.com is forwarded to a SurveyArk Inc. mailbox and used to reply to you.

Fonts.

The Audiowide and IBM Plex typefaces are served by Google Fonts from fonts.googleapis.com and fonts.gstatic.com. Your browser requests those files directly from Google, which sends your IP address and standard request headers to Google on every page of this site. That exchange is between your browser and Google, under Google's privacy terms.

Hosting.

The site runs on Cloudflare Pages. Cloudflare logs each request at its edge: IP address, user agent, referrer, timestamp, and the page requested, along with bot and security telemetry. This is how the site is delivered and protected.

Cloudflare reports aggregate traffic and security figures to SurveyArk Inc. through its dashboard, and handles the underlying request data under its own privacy terms. It may set a cookie where its security features call for one.

Your information.

To ask what is held against your email address, or to have it removed, write to info@surveyark.com.

Scope and changes

This policy covers the surveyark.com website, and describes current practice. When the practice changes, this page is updated and the date above changes with it.

It will be extended to cover the customer survey data the SurveyArk application handles, before that application launches.
